Five Oceans Acupuncture & Herb Privacy policy

Five Oceans Acupuncture and Herb collects information known as Personal Health Information (PHI). This information  is used by the acupuncturists affiliated with the business  to treat and create long-term care plans for the patients it sees. It is also used as a part of allowing patients to access files shared by practitioners and employees of the business, as well as view and update appointment information. It is stored in compliance with HIPPA and never shared with third parties. The information is kept by the business for as long as the patient has an active account with them. Financial information that is affiliated with PHI is kept for 4 years before being discarded once the patient no longer has an account with Five Oceans Acupuncture & Herb. Patients may contact the business at any point to request that any and all PHI be deleted from their devices and records and that the patient - practitioner relationship be severed.
